Physical Therapist (PT) - Summary
- Deliver direct patient care, focusing on rehabilitation, mobility, and independence.
- Collaborate with an interdisciplinary healthcare team to ensure comprehensive patient care.
- Provide clinical guidance and support to therapy assistants and staff.

Duties & Responsibilities
- Promote and practice the organization’s mission, philosophy, and care standards.
- Conduct thorough patient evaluations to assess functional needs and desired outcomes.
- Develop, implement, and regularly adjust individualized treatment plans based on patient assessments and progress.
- Deliver direct hands-on physical therapy interventions and monitor patient responses.
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient care.
- Provide direction and support to physical therapy assistants and other staff members as needed.
- Maintain accurate and timely documentation, including evaluations, treatment notes, progress reports, discharge summaries, and required signatures in accordance with company and regulatory standards.
- Serve as a clinical resource and advocate for physical therapy within the organization and the broader community.

Qualifications & Requirements
- Graduate of a CAPTE-accredited or APTA-approved Physical Therapy program.
- Current, active, and unrestricted Physical Therapist (PT) license in the state of practice.
- Minimum of one year of experience in a healthcare setting preferred.
- Ability and willingness to travel within the assigned coverage area as required by the role.
- Reliable transportation, valid driver’s license, and current auto insurance in compliance with state laws.
- Strong communication, organizational, and documentation skills.
- Commitment to maintaining high standards of professional ethics and patient confidentiality.
