Physical Therapist (PT) - Summary
- Provide high-quality physical therapy services to patients in their homes, focusing on improving functional outcomes and promoting independence.

Duties & Responsibilities
- Deliver direct patient care and therapeutic interventions according to established protocols and best practices.
- Assess patient needs, set functional goals, and monitor progress throughout the course of therapy.
- Document assessments, treatment plans, and progress notes accurately and in compliance with regulatory requirements.
- Design and modify individualized care plans, incorporating input from patients, families, and significant others as appropriate.
- Consult with physicians, nurses, and other specialists to coordinate care and optimize treatment outcomes.
- Supervise and support therapy assistants or support staff, providing direction and guidance as needed.
- Promote physical therapy awareness and serve as a resource to the community regarding rehabilitation services.
- Ensure adherence to safety standards and company policies at all times.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for practice in the state of employment.
- Minimum one (1) year of experience in a healthcare facility setting preferred.
- Ability and willingness to travel within the designated branch or office coverage area as required.
- Dependable vehicle, valid driver’s license, and current auto insurance as required by state laws.
- Physical ability to lift 50–100 lbs and work irregular hours if necessary.
