Physical Therapist (PT) - Summary
- Provide individualized, evidence-based physical therapy services to patients with diverse rehabilitation needs
- Collaborate with multidisciplinary teams to coordinate patient care and maximize rehabilitation outcomes
- Implement treatment plans that support patient goals and enhance functional performance

Duties & Responsibilities
- Conduct comprehensive patient assessments to determine physical therapy needs and set achievable goals
- Develop and implement customized treatment plans utilizing evidence-based interventions
- Educate patients and families about treatment processes, exercises, and rehabilitation techniques
- Monitor patient progress and adjust interventions as necessary to ensure optimal outcomes
- Document patient care activities accurately and in compliance with regulatory standards
- Participate in team meetings and case conferences to discuss patient progress and coordinate care
- Maintain a safe and therapeutic environment for patients and staff
- Uphold organizational values, policies, and ethical standards at all times

Qualifications & Requirements
- Graduate of an accredited Physical Therapy program
- Current/valid Physical Therapy license (or eligibility to sit for the NPTE)
- Successful completion of a background check
- Strong interpersonal and communication skills
- Commitment to evidence-based practice and lifelong learning
