Physical Therapist (PT) - Summary

Duties & Responsibilities
- Champion Elara Caring’s mission, philosophy, and standards of care in all interactions.
- Deliver direct patient care, including comprehensive assessments and evaluations of functional needs.
- Design, implement, and modify personalized plans of care based on patient evaluations and progress.
- Coordinate with interdisciplinary team members, including support staff, to ensure integrated care delivery.
- Document patient evaluations, treatment goals, and progress in accordance with organizational and regulatory standards.
- Serve as a clinical leader and resource to peers and patients within the physical therapy discipline.
- Adapt to a dynamic, physically demanding environment and manage a flexible schedule as required by patient needs.
- Travel within the assigned coverage area to provide care at patient homes.

Qualifications & Requirements
- Graduate of an APTA-accredited Physical Therapy program.
- Current, unrestricted PT license in the relevant state.
- Minimum of one year of experience in a healthcare setting preferred.
- Ability and willingness to travel within the designated coverage area as needed.
- Reliable vehicle, valid driver’s license, and required auto insurance coverage.
- Physical ability to lift, transfer, and assist patients as needed (50–100 lbs) and perform full-range body movements.
- Strong communication, documentation, and organizational skills.
