Physical Therapist (PT) - Summary- Opportunity to provide patient-centered physical therapy services in a home care setting
- Engage with a multidisciplinary team dedicated to delivering quality care
- Support patients in achieving their rehabilitation goals and improving functional outcomes

- Conduct thorough patient assessments to determine functional needs and develop customized care plans
- Provide direct physical therapy interventions, monitor progress, and adjust treatment plans as needed
- Collaborate with other healthcare professionals and engage significant others in the care process when appropriate
- Direct and supervise support staff to help patients achieve their functional goals
- Maintain accurate documentation of patient evaluations, treatment goals, and progress notes
- Ensure all records are compliant with regulatory and organizational requirements
- Act as a resource and provide leadership to staff, promoting physical therapy within the community
- Perform job duties in a physically demanding environment, including lifting and transferring patients
- May be required to work irregular hours, including on-call shifts if applicable

- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of practice
- Minimum of one (1) year experience in a healthcare facility
- Ability and willingness to travel within the assigned coverage area as required
- Dependable vehicle, valid driver’s license, and current auto insurance per state requirements
- Capable of sitting, standing, bending, lifting, and moving intermittently, including lifting 50-100 lbs
- Demonstrates strong clinical skills and a compassionate, patient-focused approach
- Collaborative team player with excellent communication abilities
- Adaptable to changing environments and able to manage a physically demanding workload
- Detail-oriented with a commitment to maintaining accurate records and compliance standards
- Motivated to contribute to an innovative, mission-driven organization
- Reports to Branch Director, Clinical Manager, Clinical Supervisor, or designee
