Physical Therapist (PT) - Summary- Provide high-quality, patient-centered physical therapy services within a home health care environment
- Evaluate patient functional status and create individualized plans of care

- Current, unrestricted Physical Therapist (PT) license in the state of practice
- Minimum of one year of experience in a healthcare setting, preferably in home health or rehabilitation
- Ability and willingness to travel up to 50% within the designated coverage area
- Valid driver’s license and proof of auto insurance
- Physical ability to lift 50-100 lbs and perform job-related physical demands
- Demonstrates strong clinical assessment and critical thinking skills
