Physical Therapist (PT) - Summary
- Provide comprehensive physical therapy services to patients in their homes
- Assess patient needs and develop individualized care plans
- Work collaboratively within a multidisciplinary healthcare team

Duties & Responsibilities
- Perform patient evaluations to determine physical therapy needs
- Develop and implement customized care plans based on patient assessments
- Provide professional physical therapy treatments in patients' homes
- Educate patients and caregivers on therapy exercises and safety precautions
- Monitor and document patient progress, making necessary adjustments to treatment plans
- Communicate effectively with interdisciplinary team members for coordinated patient care
- Adhere to all clinical policies, procedures, and legal requirements
- Travel to patient homes within the service area as required

Qualifications & Requirements
- Degree in Physical Therapy from an APTA-accredited program
- Current, active license as a Physical Therapist (PT) in the state of practice
- Minimum of one year of experience in a healthcare setting
- Strong clinical and interpersonal skills
- Ability to lift 50-100 lbs and meet the physical demands of home-based care
- Reliable transportation and ability to travel within the service area
