In this full-time role, you will be responsible for performing comprehensive physical therapy evaluations, developing and implementing individualized treatment plans, and providing direct therapeutic interventions in patients’ homes. Your expertise will support Elara Caring’s mission of delivering the right care, at the right time, in the right place. The Physical Therapist (PT) will also be responsible for accurately documenting all patient interactions, monitoring progress, and adjusting care plans as needed to ensure the best outcomes for every patient served.
Collaboration is key in this position, as you will coordinate care with other healthcare professionals, including nurses, occupational therapists, and physicians, to provide a holistic approach to patient care. You will educate patients and their caregivers on therapeutic exercises, injury prevention, and strategies to enhance mobility and safety within the home environment. Additionally, the Physical Therapist (PT) will provide supervision and guidance to support staff, ensuring high standards of care and compliance with all regulatory requirements.

Physical Therapist (PT) - Summary- Deliver direct, evidence-based physical therapy care to patients in their homes
- Conduct thorough patient assessments and develop tailored treatment plans
- Work closely with families, caregivers, and interdisciplinary healthcare teams
- Document patient progress, outcomes, and all clinical interactions
- Supervise and mentor support staff as required

- Graduate of a CAPTE or APTA-approved Physical Therapy program
- Current, unrestricted Physical Therapist (PT) license
- Minimum of one year of healthcare-related experience preferred
- Ability to perform physical job duties, including lifting and transferring patients as required
- Reliable transportation, valid driver’s license, and current auto insurance
- Willingness to travel within assigned territory as needed
