Physical Therapist (PT) - Summary- Provide comprehensive physical therapy services to patients in their homes, focusing on rehabilitation, pain management, and functional improvement.
- Develop and implement individualized care plans based on thorough assessments and patient goals.
- Collaborate with multidisciplinary teams to coordinate and optimize patient care.
- Maintain accurate and timely clinical documentation in accordance with organizational and regulatory standards.
- Educate patients and families on therapy plans, home exercise programs, and adaptive strategies.
- Assess patients’ physical abilities and limitations to determine appropriate therapy interventions.
- Deliver hands-on patient care, including therapeutic exercises, manual therapy, and functional training.
- Create, review, and adjust individualized treatment plans to ensure optimal patient outcomes.
- Work closely with caregivers, family members, and other healthcare providers to support comprehensive patient care.
- Adhere to best practices and the highest standards of physical therapy practice.
- Document patient progress, treatment plans, and outcomes accurately and efficiently.
- Provide education and support to patients, families, and the community regarding physical therapy and health promotion.
- Participate in community outreach and educational initiatives as needed.
- Safely perform patient lifting, support, and transfers as necessary.
- Travel within the assigned coverage area to deliver in-home care services.

- Graduate of an APTA-accredited Physical Therapy program.
- Current, unrestricted Physical Therapy (PT) license in good standing.
- Minimum of one (1) year of professional experience in a healthcare setting is preferred.
- Dependable transportation, valid driver’s license, and current auto insurance.
- Willingness and ability to travel within the designated coverage area to provide patient care.
- Physical ability to sit, stand, bend, lift, and move throughout the workday, including lifting up to 100 lbs as required.
- Passionate about delivering high-quality, patient-centered care.
- Strong interpersonal and communication skills, with a collaborative spirit.
- Detail-oriented, organized, and able to manage multiple tasks effectively.
- Flexible, self-motivated, and able to work independently as well as part of a team.
