Physical Therapist (PT) - Summary- Deliver evidence-based physical therapy services to home health patients.
- Promote Elara Caring’s mission and standards of care in every patient interaction.
- Serve as a resource and advocate for physical therapy within the home health community.
- Conduct comprehensive assessments of patients’ functional abilities and therapy needs.
- Develop, implement, and revise individualized care plans based on clinical evaluations.
- Provide direct patient care and therapeutic interventions in accordance with the plan of care.
- Collaborate with other healthcare professionals to ensure coordinated, holistic care.
- Document patient evaluations, treatment goals, progress, and care plans accurately and timely.
- Involve patients’ families and significant others in the care process when appropriate.
- Direct and supervise support staff to achieve patient goals and maintain standards of care.

- Graduate of an APTA-accredited Physical Therapy program.
- Current, unrestricted PT license in the state of practice.
- Minimum of one year of experience in a healthcare setting preferred.
- Ability and willingness to travel within the designated service area.
- Reliable vehicle, valid driver’s license, and current auto insurance as required by law.
- Physical ability to sit, stand, bend, lift, and assist patients as needed, including lifting up to 100 lbs.
