Physical Therapist (PT) - Summary

Duties & Responsibilities
- Conduct thorough patient assessments and design individualized treatment plans based on clinical findings and patient goals.
- Administer direct therapeutic interventions, evaluate functional outcomes, and adjust care plans as necessary.
- Document patient evaluations, treatment goals, progress notes, and maintain proper records in compliance with regulatory standards.
- Consult with other healthcare specialists to coordinate care and maximize patient recovery.
- Provide leadership and guidance to support staff, ensuring high standards of care are maintained.
- Educate patients, families, and caregivers on therapy interventions and safe practices at home.
- Uphold safety and quality standards while delivering care in a physically demanding environment.
- Contribute to the professional development of the team by serving as a mentor and resource.
- Participate in community outreach to promote awareness about physical therapy services.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted Physical Therapist (PT) license in accordance with state regulations.
- Minimum of one year of experience in a healthcare facility or similar clinical setting preferred.
- Ability and willingness to travel within the coverage area as required by patient needs.
- Reliable vehicle, valid driver’s license, and current auto insurance as required by state law.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds, including the safe positioning and transferring of patients.
