Physical Therapist (PT) - Summary- Deliver high-quality, patient-centered physical therapy services in a home healthcare setting
- Evaluate, plan, and implement personalized care plans to support patient recovery and independence
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes
- Support the mission of providing the right care, at the right time, in the right place
- Conduct comprehensive evaluations of patients’ physical abilities and limitations
- Develop and implement individualized treatment plans based on patient needs and goals
- Provide therapeutic interventions and exercises to improve mobility, strength, and function
- Educate patients and their families about the therapy process, home exercises, and ongoing care
- Document patient progress, maintain accurate records, and adjust care plans as needed
- Communicate effectively with patients, families, and team members to coordinate care
- Ensure compliance with all regulatory and organizational requirements

- Current and valid Physical Therapist (PT) license as required by state regulations
- CPR certification from the American Heart Association or American Red Cross
- Minimum of 1 year of experience in a healthcare facility preferred
- Ability to meet the physical demands of the position
- Strong communication and interpersonal skills
