Physical Therapist (PT) - Summary
- Deliver in-home physical therapy services to patients recovering from injury, surgery, or chronic conditions.
- Work collaboratively with interdisciplinary healthcare teams to develop and execute individualized care plans.
- Encourage patient participation and empower families or caregivers to assist in recovery and rehabilitation.
- Serve as a resource and leader within the home health team, promoting best practices and continuing education.
Duties & Responsibilities
- Promote the organization’s philosophy, mission, and administrative policies to ensure exceptional care quality.
- Conduct comprehensive patient assessments to evaluate functional status and rehabilitation needs.
- Develop and adjust individualized treatment plans based on ongoing patient progress and feedback from stakeholders.
- Provide direct patient care using evidence-based physical therapy techniques.
- Collaborate with physicians, nurses, and other specialists to ensure coordinated care delivery.
- Document all evaluations, treatment goals, interventions, and progress notes clearly and accurately.
- Update patient records regularly to reflect current needs, goals, and ongoing outcomes.
- Direct and supervise support staff to achieve functional goals established for each patient.
- Contribute to community outreach and serve as an advocate for physical therapy within the local healthcare community.
- Adapt to a physically demanding and high-stress environment, including lifting, transferring, and positioning patients as needed.
- May be required to work irregular hours, including on-call shifts if necessary.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for practice in the state of employment.
- Minimum of one (1) year of clinical experience in a healthcare facility, preferably in home health or rehabilitation.
- Willingness and ability to travel within the assigned service area as required for patient visits.
- Valid driver’s license, reliable transportation, and current auto insurance in compliance with state regulations.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including equipment and patient transfers.
- Strong organizational and time management skills for managing caseloads efficiently.
- Excellent communication and interpersonal skills to foster positive relationships with patients and team members.
