Physical Therapist (PT) - Summary
- Deliver high-quality, patient-focused physical therapy services in a home health environment.

- Work collaboratively with an interdisciplinary team, patients, and caregivers to maximize functional recovery.
Duties & Responsibilities
- Provide direct patient care in accordance with the plan of care and established physical therapy standards.
- Evaluate patients’ functional needs and outcomes, regularly updating individualized treatment plans.
- Design and implement comprehensive care plans based on thorough assessments, involving significant others as appropriate.
- Consult and collaborate with other healthcare specialists, ensuring a coordinated approach to patient care.
- Supervise and provide guidance to support staff, directing their efforts to help patients achieve functional goals.
- Document patient evaluations, treatment goals, plans, and progress, ensuring compliance with regulatory and organizational requirements.
- Maintain accurate records, including prescriptions and required signatures, for all therapy-related interventions.
- Lead and participate in community outreach to promote awareness of physical therapy services.
- Adhere to safety and quality standards, including handling, lifting, and transferring patients as needed.
- May be required to work irregular or on-call hours depending on patient needs.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of practice.
- Minimum of one (1) year of experience in a healthcare facility or clinical setting.
- Reliable vehicle, valid driver’s license, and up-to-date auto insurance in accordance with state regulations.
- Ability and willingness to travel within the assigned branch coverage area as needed (up to 50%).
- Physical capability to lift, push, pull, and support up to 50 pounds, and to perform all essential functions of the role.
