Physical Therapist (PT) - Summary
- Deliver high-quality physical therapy services in a home care setting, supporting patient recovery and mobility.

Duties & Responsibilities
- Provide direct patient care, including thorough assessment, evaluation of functional needs, and the development of individualized treatment plans.
- Design and implement therapy plans based on comprehensive assessments, involving patients and significant others as appropriate.
- Collaborate with other specialists and healthcare professionals to ensure optimal patient outcomes.
- Adjust and update care plans in response to patient progress and changing needs.
- Document evaluations, treatment goals, plans, and ongoing progress, maintaining all required prescriptions and appropriate signatures.
- Lead and direct support staff to assist patients in achieving functional goals.
- Serve as a resource and provide leadership to staff, promoting physical therapy within the community.
- Work in a physically demanding environment that may require handling, lifting, and transferring patients, as well as moving medical equipment.
- Maintain therapy practice within defined standards of care and organizational policies.
- Participate in call hours and work irregular schedules as required to meet patient needs.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of employment.
- Minimum of one (1) year experience in a healthcare facility, preferably in a home care setting.
- Ability and willingness to travel within the designated branch or office coverage area.
- Possession of a dependable vehicle, valid driver’s license, and current auto insurance as required by law.
- Capacity to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning and equipment transport.
