Physical Therapist (PT) - Summary
- Responsible for evaluating patients' needs, developing individualized treatment plans, and delivering evidence-based physical therapy interventions.
- Collaborates with interdisciplinary teams to optimize patient outcomes and supports the organization's mission of high-quality care.
- Provides leadership, direction, and support to therapy staff and acts as a clinical resource within the organization.
Duties & Responsibilities
- Promote the organization’s philosophy, mission, and administrative policies to maintain quality standards of care.
- Provide direct patient care including assessment, treatment planning, and evaluation of functional outcomes.
- Collaborate with other specialists and consult on patient care as necessary to enhance recovery.
- Design and implement individualized plans of care, incorporating family or significant others when appropriate.
- Practice within defined standards of care for physical therapy.
- Direct and guide support staff to achieve patients’ functional goals.
- Document evaluations, treatment goals, care plans, and maintain all required prescriptions and signatures.
- Act as a resource for physical therapy within the organization and the broader community.
- Work in a physically demanding environment requiring full range of body motion, including lifting and transferring patients.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapy license in the state of practice.
- Minimum of one year of experience in a healthcare facility providing physical therapy services.
- Ability and willingness to travel within assigned coverage area as needed.
- Reliable vehicle, valid driver’s license, and current auto insurance as required by state law.
- Ability to lift, push, pull, and support up to 50 pounds as part of daily job duties.
