Physical Therapist (PT) - Summary
- Evaluate, plan, and implement individualized patient treatment programs.
- Restore, maintain, and promote optimal physical function and wellness.
- Collaborate with patients, families, and healthcare teams to develop and monitor goals.
- Support patients throughout the rehabilitation process in a home health setting.
Duties & Responsibilities
- Conduct comprehensive physical therapy assessments and develop appropriate treatment plans tailored to each patient’s needs.
- Administer therapeutic exercises, manual therapy, and modalities as indicated.
- Monitor and document patient progress, adjusting interventions as necessary to achieve optimal outcomes.
- Educate patients and caregivers on exercises and techniques to support recovery and independence.
- Communicate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated, quality care.
- Maintain accurate and timely clinical records in accordance with regulatory standards and organizational policies.
- Ensure safety and compliance with all regulatory and professional standards during patient care activities.
- Utilize reliable transportation to travel to patient homes as required for service delivery.

Qualifications & Requirements
- Current, valid Physical Therapist (PT) license as required by state regulations.
- CPR certification from the American Heart Association or American Red Cross.
- Minimum of one year of experience working in a healthcare facility or similar setting.
- Strong ability to meet the physical demands of the job, including lifting and assisting patients as needed.
- Demonstrated commitment to quality patient care and professional ethics.
- Excellent verbal and written communication skills for effective collaboration with patients and healthcare teams.
