Physical Therapist (PT) - Summary

Duties & Responsibilities
- Conduct thorough patient assessments to determine physical therapy needs and establish baseline function.
- Develop and implement customized plans of care in collaboration with patients, families, and healthcare providers.
- Provide hands-on therapeutic interventions, including exercises, manual techniques, and patient education.
- Evaluate, document, and track patient progress towards established goals using appropriate clinical standards.
- Maintain accurate and timely records, including treatment plans, progress notes, and physician orders.
- Collaborate with nurses, physicians, and other therapists to ensure comprehensive, coordinated care.
- Supervise and provide guidance to physical therapy support staff as needed.
- Act as a resource and advocate for physical therapy in the community and within the healthcare team.
- Ensure compliance with all regulatory and organizational standards for quality and safety.
- Perform duties in a physically demanding environment, requiring safe patient handling, lifting, and transferring.

Qualifications & Requirements
- Graduate of an accredited physical therapy program approved by the American Physical Therapy Association (APTA).
- Current, unrestricted Physical Therapist (PT) license valid in the state of employment.
- Minimum of one year of experience in a healthcare facility, preferably with home health or outpatient experience.
- Ability and willingness to travel regularly within the service area to provide in-home care.
- Reliable transportation, a valid driver’s license, and up-to-date auto insurance.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds as required for patient care.
- Strong interpersonal and communication skills, with a commitment to collaborative care.
