Physical Therapist Assistant (PTA) Home Health - Summary- Deliver physical therapy services under the supervision of a licensed Physical Therapist in a home health setting.
- Promote Elara Caring’s mission and philosophy by ensuring the highest standards of patient care.
- Assist patients in achieving their rehabilitation goals within their home environment.
- Provide direct physical therapy interventions as outlined in the patient’s individualized Plan of Care, established by the supervising Physical Therapist and certified by the patient’s physician.
- Verify and review the Plan of Care prior to each visit, ensuring care is delivered according to physician orders, assessment data, and established clinical guidelines.
- Monitor and report any changes in the patient’s clinical status to the supervising Physical Therapist and other care team members as required.
- Adhere to State Practice Act guidelines and maintain compliance with all regulatory and organizational policies.
- Incorporate patient-centered goals into therapy sessions, consistently working towards the achievement of desired outcomes.
- Educate patients and their families on the safe and effective use of prosthetic, orthotic, and assistive devices.
- Provide instruction on home therapy programs, empowering patients and caregivers to participate actively in ongoing rehabilitation.
- Maintain accurate and timely documentation of all patient interactions, interventions, and progress.

- Graduation from an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association.
- Current, unrestricted PTA license valid for practice in the applicable state.
- Minimum of six months’ experience as a Physical Therapist Assistant preferred; new graduates are encouraged to apply.
- Ability and willingness to travel within the assigned branch or office coverage area.
- Reliable transportation, valid driver’s license, and current automobile insurance in accordance with state regulations.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment movement.
- Demonstrates compassion, empathy, and a patient-centered approach to care.
- Possesses strong interpersonal and communication skills for effective collaboration with patients, families, and interdisciplinary teams.
