As a Physical Therapist Assistant (PTA) in Home Health, you will work closely with supervising Physical Therapists and interdisciplinary clinical teams to implement individualized treatment plans and ensure that all therapy goals are met with compassion and clinical expertise. Physical Therapist Assistant (PTA) Home Health - Summary
- Deliver physical therapy services under the supervision of a licensed Physical Therapist in a home health setting.

Duties & Responsibilities
- Provide direct physical therapy interventions as outlined in the patient’s individualized Plan of Care, established by the supervising Physical Therapist and certified by the patient’s physician.
- Verify and review the Plan of Care prior to each visit, ensuring care is delivered according to physician orders, assessment data, and established clinical guidelines.
- Monitor and report any changes in the patient’s clinical status to the supervising Physical Therapist and other care team members as required.
- Adhere to State Practice Act guidelines and maintain compliance with all regulatory and organizational policies.
- Incorporate patient-centered goals into therapy sessions, consistently working towards the achievement of desired outcomes.
- Educate patients and their families on the safe and effective use of prosthetic, orthotic, and assistive devices.
- Provide instruction on home therapy programs, empowering patients and caregivers to participate actively in ongoing rehabilitation.
- Maintain accurate and timely documentation of all patient interactions, interventions, and progress.

Qualifications & Requirements
- Graduation from an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association.
- Current, unrestricted PTA license valid for practice in the applicable state.
- Minimum of six months’ experience as a Physical Therapist Assistant preferred; new graduates are encouraged to apply.
- Ability and willingness to travel within the assigned branch or office coverage area.
- Reliable transportation, valid driver’s license, and current automobile insurance in accordance with state regulations.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment movement.
