Physical Therapist Assistant (PTA) – Home Health Summary
- Deliver safe, effective, and compassionate physical therapy services to patients in their homes in accordance with established Plans of Care.
- Work collaboratively with Physical Therapists, physicians, and interdisciplinary care teams to ensure continuity of care.

Duties & Responsibilities
- Provide direct patient care as prescribed by the supervising Physical Therapist and patient’s physician, following all established protocols and standards of practice.
- Implement therapy interventions based on the individualized Plan of Care and document patient progress accurately and thoroughly.
- Monitor and report any significant changes in patient status or response to treatment to the supervising Physical Therapist and healthcare team.
- Educate patients and their families/caregivers on home exercise programs, safety in the use of prosthetic, orthotic, and assistive devices, and therapeutic activities that support rehabilitation goals.
- Adhere to all applicable state regulations, professional guidelines, and physician orders in the provision of therapy services.
- Maintain timely and effective communication with all involved care providers to ensure seamless patient care transitions.
- Perform physical tasks such as lifting, transferring, and assisting patients, ensuring safety and comfort at all times.
- May be required to work irregular or on-call hours in response to patient needs or as scheduled by the care team.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant (PTA) program recognized by the American Therapy Association.
- Current, unrestricted Physical Therapist Assistant (PTA) license valid for the state of employment.
- At least six months of experience as a Physical Therapist Assistant (PTA) preferred, with prior home health experience considered an asset.
- Ability and willingness to travel within the assigned area as required by patient caseload and scheduling needs.
- Reliable vehicle, valid driver’s license, and current auto insurance are essential for the home health aspect of the role.
