As a Physical Therapist Assistant (PTA) Home Health, you will provide direct therapy in accordance with the established Plan of Care and under the supervision of a licensed Physical Therapist. Physical Therapist Assistant (PTA) Home Health - Summary
- Deliver direct physical therapy services to patients in their homes, following the established Plan of Care and under the supervision of a licensed Physical Therapist.
- Promote patient safety, well-being, and optimal functional outcomes through effective instruction, support, and therapy techniques.
- Work collaboratively within a multidisciplinary team to ensure high-quality, individualized patient care.
Duties & Responsibilities
- Promote Elara Caring’s mission and adhere to administrative policies to maintain the highest standards of care.
- Perform direct physical therapy services in accordance with physician certification, accepted standards, and patient-specific goals.
- Follow, verify, and update the patient’s Plan of Care as established by the supervising Physical Therapist.
- Provide instruction on the use of prosthetic, orthotic, and assistive devices, tailoring education to each patient’s needs.
- Comply with all applicable State Practice Act regulations and guidelines to ensure legal and ethical practice.
- Instruct patients and caregivers on home therapy programs, ensuring clear communication and understanding to promote adherence and safety.
- Assist with patient transfers and mobility, and work in a physically demanding environment that may require frequent lifting and movement.
- May be required to work irregular hours and be available for on-call responsibilities as needed to meet patient requirements.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant (PTA) program approved by the American Therapy Association or equivalent accrediting body.
- Current, unrestricted PTA license valid in the state of practice.
- Minimum of six months of experience as a Physical Therapist Assistant (PTA) preferred; new graduates with strong clinical skills are encouraged to apply.
- Willingness and ability to travel within the designated branch or office coverage area as required (up to 50%).
- Must possess a reliable vehicle, valid driver’s license, and current auto insurance for travel to patient homes.
- Physical ability to frequently lift, push, pull, or support up to 50 pounds as part of patient care activities.
