Physical Therapist Assistant (PTA) – Home Health Summary
- Delivers direct physical therapy services in accordance with established care plans under the supervision of a licensed Physical Therapist.
- Provides in-home rehabilitative care to diverse patient populations, focusing on recovery, mobility, and independence.

Duties & Responsibilities
- Perform physical therapy interventions as outlined in the Plan of Care and certified by the patient’s physician to ensure quality and continuity of care.
- Monitor and report any changes in patient status to the supervising Physical Therapist.
- Adhere to State Practice Act Guidelines and verify the Plan of Care prior to each visit.
- Integrate patient care goals into therapy sessions and adjust care as needed for optimal outcomes.
- Instruct patients and families in the safe, appropriate use of prosthetic, orthotic, and assistive devices.
- Educate patients and caregivers on the therapy home program to support ongoing progress between visits.
- Perform physically demanding tasks, including lifting, transferring, and supporting patients, as well as moving equipment as necessary.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association (APTA).
- Current, unrestricted PTA license valid for the applicable state.
- At least 6 months of experience as a Physical Therapist Assistant preferred, though not required.
- Ability and willingness to travel within the assigned coverage area as needed (up to 50%).
- Reliable transportation, valid driver’s license, and current auto insurance in accordance with state laws.
- Physical ability to lift, push, pull, and support up to 50 pounds, including patient positioning and equipment handling.
