Physical Therapist Assistant (PTA) - Home Health Summary- Deliver in-home physical therapy care to patients under the supervision of a Physical Therapist
- Support patient recovery and enhance mobility, independence, and quality of life
- Ensure adherence to individualized care plans and regulatory standards
- Administer direct physical therapy services in accordance with physician-certified care plans
- Monitor, document, and communicate patient progress and status changes to the supervising Physical Therapist
- Adhere to state practice guidelines and maintain accurate, timely documentation
- Incorporate patient-centered goals into therapy sessions for measurable outcomes
- Educate patients and caregivers on safe use of assistive devices and home therapy techniques
- Safely lift, transfer, and support patients as needed (up to 50 pounds)
- Travel within assigned area to provide care in patient homes

- Graduate of an accredited Physical Therapist Assistant program (approved by the American Therapy Association)
- Current, unrestricted PTA license valid in the state of employment
- Preferred: at least 6 months of prior experience as a Physical Therapist Assistant
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability and willingness to travel within the assigned coverage area
- Physical capability to lift, transfer, and assist patients safely
- Compassionate and patient-focused with strong interpersonal skills
- Excellent communicator, able to collaborate with multidisciplinary teams
- Organized, detail-oriented, and committed to accurate documentation
- Adaptable and able to manage travel and scheduling within the coverage area
