The Physical Therapist Assistant (PTA) – Home Health is instrumental in delivering hands-on physical therapy services to patients in their homes. This role involves following individualized plans of care designed by licensed Physical Therapists, reporting patient progress, and adjusting care as directed. As a vital member of the home health team, you will provide education to patients and caregivers, encourage compliance with therapy regimens, and support the overall rehabilitation process. This position requires travel within the designated coverage area and a high degree of flexibility to meet patients’ needs, including potential irregular hours.
Duties & Responsibilities
- Deliver direct physical therapy interventions as outlined in the patient’s individualized Plan of Care under the supervision of a licensed Physical Therapist
- Promote the organization’s philosophy, mission, and administrative policies to ensure quality and continuity of patient care
- Adhere strictly to State Practice Act guidelines and maintain compliance with industry standards and regulations
- Verify and review the Plan of Care prior to each patient visit, ensuring that all therapeutic activities align with physician orders and assessment data
- Monitor and report any changes in patient status or response to therapy promptly to the supervising Physical Therapist
- Incorporate established therapy goals into each patient session and document progress consistently
- Educate patients and their families/caregivers about the safe use of prosthetic, orthotic, and assistive devices
- Instruct patients and caregivers on executing home therapy programs to promote independence and recovery
- Maintain accurate and timely records of patient care activities, interventions, and outcomes
- Perform patient handling, lifting, and transfers as needed, ensuring patient safety and comfort at all times
- Demonstrate flexibility by working irregular hours, including possible on-call assignments, as required by patient needs

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association (APTA)
- Current, unrestricted Physical Therapist Assistant (PTA) license valid in the state of practice
- At least 6 months of experience as a Physical Therapist Assistant, preferably in a home health or similar setting
- Ability and willingness to travel within the designated branch or office coverage area
- Reliable transportation, valid driver’s license, and current auto insurance are required
- Physical capability to frequently lift, push, pull, and support up to 50 pounds, and perform full range of body motion
