Duties & Responsibilities
- Deliver direct physical therapy services to patients in home settings in accordance with the Plan of Care and physician certification.
- Uphold the company’s philosophy, mission, and policies to ensure the highest quality of care for every patient.
- Consistently follow the Plan of Care as developed by the supervising physical therapist, reporting any changes in patient status promptly.
- Verify the Plan of Care before each visit and adhere strictly to State Practice Act guidelines and regulatory requirements.
- Integrate individualized patient care goals into therapy sessions and instruct patients and caregivers on home exercise programs.
- Educate patients and families on the safe and effective use of assistive devices such as walkers, canes, and wheelchairs.
- Maintain accurate and timely documentation of patient progress, treatments, and outcomes as required by company policies.
- Participate in ongoing professional development and training opportunities as directed by supervisors or regulatory bodies.
- Work collaboratively with physical therapists, nurses, and other healthcare professionals to optimize patient care.
- Adapt to a physically demanding environment, including lifting, transferring, and supporting patients during therapy activities.
- Be available for irregular or on-call hours as needed to meet patient care demands.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant (PTA) program approved by the American Therapy Association.
- Current and unrestricted Physical Therapist Assistant (PTA) license for the state of practice.
- Minimum of 6 months experience as a Physical Therapist Assistant preferred.
- Ability and willingness to travel within the assigned branch coverage area (up to 50%).
- Reliable vehicle, valid driver’s license, and current auto insurance required.
- Physical ability to lift, push, pull, and support up to 50 pounds as part of patient care activities.
- Strong communication, interpersonal, and organizational skills.
- Commitment to maintaining patient confidentiality and adhering to all regulatory and ethical standards.
