Physical Therapist Assistant (PTA) - Home Health Summary
The Physical Therapist Assistant (PTA) - Home Health collaborates with physical therapists to deliver hands-on therapy interventions that align with the patient’s individualized plan of care. This position is essential in supporting patients as they work towards regaining mobility, independence, and improved quality of life. As a PTA in home health, you will be responsible for delivering therapeutic exercises, educating patients and caregivers, and ensuring compliance with all relevant guidelines and safety protocols. The role requires flexibility, adaptability, and a commitment to maintaining the highest standards of patient care within the home setting.
Duties & Responsibilities
- Promote the organization's philosophy, mission, and administrative policies to foster high-quality patient care.
- Deliver direct physical therapy services in accordance with established standards and the patient’s physician-certified plan of care.
- Follow the plan of care as established and supervised by the Physical Therapist, reporting any changes in patient status promptly.
- Adhere strictly to State Practice Act Guidelines and verify the appropriateness of the plan of care before each visit.
- Provide care based on physician orders, assessment data, and established therapeutic standards and guidelines.
- Integrate patient care goals into therapy sessions and instruct patients and/or families on the safe use of adaptive and assistive devices.
- Offer comprehensive patient and caregiver instruction in effective home therapy programs to support ongoing progress.
- Maintain the physical ability necessary for a full range of body motion, including lifting and transferring patients as needed.
- Work irregular hours, including on-call hours, as required to meet patient and operational needs.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant program approved by the American Therapy Association.
- Current, unrestricted PTA license valid in the state of practice.
- Six months or more of clinical experience as a Physical Therapist Assistant preferred.
- Ability and willingness to travel within the assigned branch coverage area, up to 50% of the time.
- Valid driver’s license, dependable vehicle, and current auto insurance in accordance with state requirements.
- Physical capacity to lift, push, pull, and support up to 50 pounds as part of routine patient care activities.
