Physical Therapist Assistant (PTA) - Summary- Deliver direct physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist.
- Support patients as they work toward their rehabilitation and functional mobility goals in a one-on-one, home-based setting.
- Implement and monitor individualized Plans of Care, ensuring adherence to physician orders and industry standards.
- Communicate regularly with supervising therapists, patients, and families to optimize care outcomes and satisfaction.
- Provide high-quality, evidence-based physical therapy services in accordance with established treatment plans and physician directives.
- Promote the organization’s philosophy, mission, and policies to ensure the highest standard of care for all patients.
- Execute all aspects of the Plan of Care as directed by the supervising Physical Therapist, reporting significant changes in patient condition promptly.
- Verify and review the Plan of Care prior to each visit to ensure accuracy and compliance with physician orders.
- Incorporate patient-specific goals into therapy sessions, documenting all interventions and progress in a timely and thorough manner.
- Educate patients and their families on safe mobility techniques, including the proper use of assistive devices and home exercise programs.
- Maintain a full range of body motion to assist with safe patient transfers and provide hands-on care in various physical environments.

- Graduate of an accredited physical therapist assistant program recognized by the American Physical Therapy Association (APTA).
- Current, unrestricted PTA license valid in the state of employment.
- At least 6 months of prior experience as a Physical Therapist Assistant is preferred, especially in home health or similar settings.
- Willingness and ability to travel within the assigned coverage area as needed for patient visits.
- Valid driver’s license, reliable vehicle, and up-to-date auto insurance for travel requirements.
- Physical ability to lift 50-100 lbs, stand, bend, and move intermittently throughout the workday.
- Demonstrates a strong commitment to patient-centered care and upholds high ethical standards in practice.
