Physical Therapist Assistant (PTA) - Summary
- Assist licensed Physical Therapists in providing therapeutic care to patients in a home setting
- Support patients’ rehabilitation and recovery through evidence-based treatment interventions
- Promote patient safety, independence, and quality of life through education and hands-on care
- Work collaboratively with interdisciplinary teams to achieve optimal patient outcomes
Duties & Responsibilities
- Deliver direct physical therapy services as directed by the supervising Physical Therapist and consistent with the patient’s individualized plan of care
- Promote and adhere to Elara Caring’s mission, philosophy, and administrative policies to ensure quality and continuity of care
- Verify and review the plan of care prior to each patient visit, ensuring compliance with physicians’ orders and state regulations
- Document all care provided according to agency guidelines and standards of practice
- Instruct patients and caregivers on the safe and proper use of prosthetic, orthotic, and assistive devices such as canes, walkers, and wheelchairs
- Educate patients and family members about home exercise programs to encourage continued improvement and recovery
- Monitor and promptly report any changes in patient status to the supervising Physical Therapist
- Comply with relevant state practice act guidelines and maintain safety in all care activities
- Perform physical tasks including patient transfers, lifting, and equipment management as required for patient care
- Participate in team meetings and collaborate with other healthcare professionals to support patient goals

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant (PTA) program, approved by the American Physical Therapy Association
- Current, unrestricted PTA license valid for the state of practice
- At least 6 months of experience as a Physical Therapist Assistant preferred
- Ability and willingness to travel within the assigned branch or office coverage area
- Reliable vehicle, valid driver’s license, and current automobile insurance, as required by state law
- Physical ability to frequently lift, push, pull, and support weights up to 50 pounds, including assisting with patient transfers and moving equipment
