As a Physical Therapist Assistant (PTA), you will work under the supervision of licensed Physical Therapists to ensure the delivery of effective therapy interventions aligned with individualized patient care plans. Physical Therapist Assistant (PTA) - Summary
- Support delivery of physical therapy services in a home health setting.
- Implement and monitor therapy care plans in collaboration with Physical Therapists.
- Educate and empower patients and caregivers to achieve optimal mobility and independence.
- Adhere to all state, federal, and company regulations for home health care.
- Contribute to the mission of providing high-quality, compassionate care to every patient.
Duties & Responsibilities
- Promote the organization’s philosophy, mission, and administrative policies to ensure consistent quality of care.
- Deliver direct physical therapy services in accordance with accepted standards of practice and patient physician certifications.
- Follow and implement Plan of Care established by the supervising Physical Therapist, reporting any changes in patient status promptly.
- Uphold State Practice Act guidelines and verify Plan of Care before each visit.
- Integrate patient care goals into all therapy interventions and document progress.
- Instruct patients and families in safe and appropriate use of assistive devices, such as canes, walkers, and wheelchairs.
- Educate patients and caregivers on home exercise programs and support them in carrying out therapy plans independently.
- Support patient mobility through safe handling, lifting, and transfer techniques as needed in a physically demanding environment.
- Collaborate with multidisciplinary team members to ensure continuity of care and optimal outcomes.
- Maintain accurate, timely documentation and comply with all regulatory requirements.

Qualifications & Requirements
- Graduate of an accredited physical therapy assistant program approved by the American Therapy Association.
- Current, unrestricted PTA license valid for the state of practice.
- Preferred minimum of 6 months’ experience as a Physical Therapist Assistant.
- Ability and willingness to travel within the designated branch/office coverage area.
- Reliable transportation, valid driver’s license, and current auto insurance in accordance with state laws.
- Physical ability to sit, stand, bend, lift, and move intermittently and to lift 50-100 lbs.

Other Relevant Information
- Reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designated supervisor.
- May require irregular hours, including on-call shifts as appropriate.
