Physical Therapist Assistant (PTA) - Summary
- Work as part of a committed team of healthcare professionals to deliver home-based physical therapy services.
- Implement care plans and therapeutic interventions as established by licensed physical therapists.
- Promote patient independence and facilitate recovery through evidence-based practices.

Duties & Responsibilities
- Deliver direct physical therapy services in accordance with the plan of care and under the supervision of a licensed physical therapist.
- Promote Elara Caring’s philosophy, mission statement, and policies to ensure the highest quality of patient care.
- Verify the plan of care before each visit and provide care based on physician’s orders, assessment data, and established guidelines.
- Adhere to applicable state practice act guidelines for physical therapy assistants.
- Incorporate patient care goals into therapy sessions and adjust interventions based on patient progress and feedback.
- Educate patients and their families on the safe and effective use of prosthetic, orthotic, and assistive devices such as canes, walkers, and wheelchairs.
- Instruct patients and caregivers in home exercise programs to support ongoing rehabilitation.
- Document patient care activities, progress, and outcomes accurately and promptly.
- Communicate changes in patient status to the supervising physical therapist and collaborate on necessary care adjustments.
- Support patient safety and comfort while handling, lifting, and transferring patients as needed.
- Maintain compliance with all regulatory and organization standards.
- Travel within the designated coverage area to provide in-home therapy services.
- Participate in ongoing education and professional development opportunities.

Qualifications & Requirements
- Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association (APTA).
- Current, unrestricted license as a Physical Therapist Assistant (PTA).
- At least six months' experience as a Physical Therapist Assistant is preferred.
- Ability and willingness to travel within the designated coverage area as required.
- Must possess a dependable vehicle, valid driver’s license, and current auto insurance according to state requirements.
- Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and equipment movement.
- Ability to work in a physically demanding and sometimes high-stress environment.
- Willingness to work irregular hours, including on-call shifts if needed.
