As a Physical Therapist Assistant (PTA), you will:

- Deliver physical therapy interventions to support patient goals and uphold continuity of care.
- Follow the established Plan of Care developed by the supervising Physical Therapist and comply with State Practice Act Guidelines.
- Review and verify the Plan of Care prior to each patient visit and carry out care in accordance with physician’s orders and best practice protocols.
- Educate patients and their families on the safe use of assistive devices and implementation of therapy-based home exercise programs.
- Demonstrate the physical ability to safely handle, lift, and transfer patients as required for effective treatment.
- Report any significant changes in patient status promptly to the supervising Physical Therapist and healthcare team.
- Maintain accurate, timely, and thorough documentation of all care provided in line with regulatory and organizational standards.
- Collaborate with interdisciplinary healthcare professionals to ensure a seamless continuum of care for each patient.
- Travel within the assigned coverage area to provide home-based therapy services.

Duties & Responsibilities
- Administer physical therapy interventions as outlined in the patient’s Plan of Care.

- Educate patients and caregivers on exercise routines and equipment usage for ongoing progress at home.
- Participate in case conferences and team meetings to coordinate comprehensive patient care.
- Uphold safety standards and infection control protocols throughout all interactions with patients and families.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant (PTA) program approved by the American Physical Therapy Association (APTA).
- Current, unrestricted PTA license in the state of practice.
- Minimum of six months’ experience as a Physical Therapist Assistant preferred.
- Ability and willingness to travel within the defined coverage area.
- Reliable transportation, valid driver’s license, and current auto insurance required.
- Physical capability to lift and support up to 50 pounds as part of patient care responsibilities.
