As a Physical Therapist, youll contribute to delivering high-quality care in a home-based environment. Responsibilities include:
- Providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists.
- Designing and implementing individualized care plans based on thorough assessments.
- Conducting therapy within established standards and directing support staff to achieve patient functional goals.
- Documenting evaluations, treatment goals, and care plans and maintaining required prescriptions and signatures.
- Promoting physical therapy services within the community and providing leadership to fellow staff.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare facility.
- Reliable vehicle, valid drivers license, and current auto insurance.
- Able and willing to travel within branch/office coverage area, up to 50%.
