The Patient Navigator (CPN) plays a vital role in facilitating access to health, wellness, and ancillary services for patients and the broader community. This position is designed to provide advocacy, information, enrollment assistance, and follow-up for individuals navigating Massachusetts Health Care reform and related benefit programs. As a Patient Navigator (CPN), you will serve as a critical resource for individuals seeking assistance with insurance enrollment, eligibility, and troubleshooting of billing issues, ensuring a seamless experience for patients interacting with healthcare systems. This role is grant-funded and may not extend beyond the grant period.
Patient Navigator (CPN) - Summary
- Advocates for patients and community members in accessing available health and wellness services.
- Provides expert guidance with enrollment in Massachusetts Health Care programs, including MassHealth, Commonwealth Care, and the Health Safety Net.
- Supports individuals through insurance reinstatement, billing issues, and transitions between health plans.
- Acts as a key point of contact for patients seeking information and assistance related to healthcare access.
Duties & Responsibilities
- Responds promptly to requests for assistance with insurance application and enrollment from both new and existing patients.
- Guides patients through the process of applying for state-subsidized programs using the Health Insurance Exchange website.
- Collaborates with clients and external agencies to confirm activation and continuity of insurance coverage.
- Participates in community outreach and enrollment events as needed, representing the organization and providing enrollment support to the public.
- Prepares and submits monthly reports and statistics on enrollment and referrals for grant documentation and compliance.
- Maintains up-to-date knowledge of state healthcare benefits, regulations, and enrollment processes by attending conferences, forums, and training sessions.
- Provides support to patients facing issues with medical groups, hospitals, or social organizations, ensuring comprehensive assistance.
- Works collaboratively with other organizations to support mutual patients and ensure continuity of care.
- Answers inquiries regarding eligibility guidelines, application requirements, and services, both in person and by phone, delivering high-quality customer service.

Qualifications & Requirements
- Must obtain and maintain annual Certified Patient Navigator (CPN) certification as required by the State of Massachusetts.
- Demonstrated ability to analyze billing information and implement process improvements based on business needs.
- Ability to research and interpret complex healthcare regulations effectively.
- Strong analytical, reading, and comprehension skills in interpreting scientific, technical, financial, and legal documents.
- Proficient in responding to inquiries or complaints from patients, regulatory agencies, or members of the business community.
- Competence in calculating figures and managing data accurately.
- Proven problem-solving skills and adaptability in non-standardized situations.
- Familiarity with Microsoft Office applications, including Outlook, Word, PowerPoint, and Excel.
- Associate degree preferred, or equivalent combination of education and two or more years of direct experience in a related field.
