Through excellent communication skills and keen attention to detail, you will be instrumental in helping patients navigate their medical appointments, understand the importance of continuity of care, and stay informed about essential visits such as Medicare Annual Wellness Visits.
In addition to handling daily scheduling and registration tasks, the Patient Coordinator will proactively reach out to patients to schedule follow-up appointments, address gaps in care, and remind patients of upcoming visits. Patient Coordinator - Summary
- Serve as the primary liaison between patients and the healthcare practice, ensuring a positive and efficient experience.

Duties & Responsibilities
- Answer incoming calls with professionalism and a helpful attitude.
- Verify and update patient information for each interaction.
- Schedule patient appointments according to provider templates and provide clear instructions for appointment preparation.
- Ensure complete and accurate registration data, including insurance and demographic information, is entered into the EMR system.
- Assist patients in selecting a primary clinician and educate them on the benefits of continuity of care.
- Initiate calls to patients needing follow-up appointments and schedule accordingly.
- Coordinate the scheduling of appointments to address gaps in care and for Medicare Annual Wellness Visits.
- Send appointment reminders via email or phone for high-priority visits.
- Contact patients who have missed appointments (No-Shows) to reschedule.
- Collaborate with providers and the Practice Administrator to resolve scheduling conflicts.
- Reschedule appointments as needed due to changes in provider availability.
- Attend staff meetings and participate in ongoing education as required by the practice.
- Work as part of a team to ensure exceptional patient service and satisfaction.
- Maintain strict confidentiality and adhere to all HIPAA regulations.
- Perform other duties as assigned.
