Nurse Practitioner (NP) - Summary

Duties & Responsibilities
- Conduct comprehensive assessments for adult and geriatric patients across various care settings, including clinics, homes, senior living residences, and via telehealth
- Manage transitions of care to minimize avoidable hospitalizations and emergency department visits
- Identify care gaps and collaborate closely with primary care providers and other health professionals to develop and optimize individualized care plans
- Evaluate and manage acute, non-life-threatening medical conditions
- Support chronic disease management, preventive care initiatives, and advanced care planning to promote long-term health and wellness
- Participate in interdisciplinary team meetings to ensure coordinated care for patients with complex health needs

Qualifications & Requirements
- Current and unrestricted Nurse Practitioner (NP) license in Oregon
- Board certification by ANCC or AANP
- Three to five years of experience as a Nurse Practitioner in primary care, geriatrics, internal medicine, family medicine, or palliative care (preferred)
- Current DEA licensure or eligibility to obtain
- Active CPR/BLS certification
- Spanish bilingual skills a plus, but not required
