Nurse Practitioner – Primary Care (NP, FNP, AGNP) Summary- Full-time, on-site position in Idaho Falls, Idaho
- Join a newly established primary care clinic focused on value-based care for adults and seniors
- Collaborative, NP-led team environment
- Emphasis on prevention, chronic disease management, and coordinated care
- Deliver comprehensive, holistic primary care to adult and senior patients in an outpatient clinic setting
- Manage a dedicated panel of Medicare Advantage and senior patients, ensuring quality and continuity of care
- Conduct wellness visits, health risk assessments, and provide chronic disease management
- Coordinate care with internal team members and external healthcare partners to optimize patient outcomes
- Participate in quality improvement initiatives and clinical team meetings to enhance practice standards
- Support the launch and growth of a new primary care clinic, helping to establish operational excellence and a positive clinic culture
- Foster meaningful, long-term patient relationships

- Current DEA license
- Valid CPR/BLS certification
