Nurse Practitioner – Primary Care (NP, Family Medicine) Summary
- Deliver high-quality, direct patient care across the lifespan within a Family Medicine Clinic setting.
- Collaborate with a multidisciplinary health team to develop and implement individualized care plans.
- Serve as a clinical resource through patient education, consultation, and support.
- Ensure adherence to evidence-based protocols and best practices for optimal patient outcomes.
- Participate in ongoing professional development and quality improvement initiatives.
Duties & Responsibilities
- Provide comprehensive primary care services to patients, including assessments, diagnosis, treatment, and follow-up care.
- Develop and manage care plans for acute and chronic medical conditions in collaboration with physicians, nurses, and other healthcare professionals.
- Conduct patient education on preventive health measures, wellness, and disease management.
- Utilize electronic medical records to document all patient interactions, care plans, and treatments accurately.
- Coordinate and facilitate referrals to specialists and ancillary services as needed.
- Participate in clinical meetings, interdisciplinary rounds, and case conferences to optimize patient care delivery.
- Adhere to organizational policies, regulatory requirements, and industry standards.
- Engage in continuous process improvement to enhance patient experiences and outcomes.

Qualifications & Requirements
- Active and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licenses.
- Current Nurse Practitioner Furnishing (NPF) license number from the California Board of Registered Nursing (BRN).
- Current board certification as a Nurse Practitioner by a nationally accredited organization in a population focus.
- Drug Enforcement Administration (DEA) certificate for controlled substances (II-V).
- National Provider Identifier (NPI).
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications if assigned to the Emergency Department.
- Demonstrated primary care experience in a clinical setting.
