Nurse Practitioner – Primary Care (NP, APRN) Summary
- Provide virtual primary care to a broad patient base across Arizona as a Nurse Practitioner – Primary Care (NP, APRN), utilizing telehealth platforms for both phone and video consultations.
- Deliver advanced chronic disease management, preventive health screenings, and palliative care support in collaboration with a supervising physician and interdisciplinary team.
- Enjoy a Monday through Friday schedule with no nights, weekends, or on-call responsibilities, ensuring a healthy work-life balance.
Duties & Responsibilities
- Conduct comprehensive virtual patient assessments and develop tailored care plans to meet individual patient needs as the Nurse Practitioner – Primary Care (NP, APRN).
- Provide expert management of chronic conditions such as diabetes, hypertension, COPD, and heart disease.
- Deliver preventive care services, including screenings, counseling, and health education to promote wellness.
- Offer palliative care support and coordinate with facility partners to enhance patient comfort and quality of life.
- Order, review, and interpret diagnostic tests; prescribe medications and treatments within the scope of practice for a Nurse Practitioner – Primary Care (NP, APRN).
- Document patient encounters accurately and efficiently in the electronic medical record (EMR).
- Collaborate with supervising physicians, specialists, and interdisciplinary team members to optimize patient care plans.

Salary & Benefits
- Competitive base salary with total potential up to $145,000, depending on experience and qualifications for the Nurse Practitioner – Primary Care (NP, APRN) role.

Qualifications & Requirements
- Master’s Degree in Nursing (MSN) from an accredited program.
- Current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP, APRN) licenses in Arizona.
- At least one year of experience as a Nurse Practitioner – Primary Care (NP, APRN), with a background in primary care, geriatrics, or palliative care preferred.
- Previous experience with telemedicine or virtual care platforms is highly desirable.
- Strong clinical judgment, effective communication skills, and a commitment to delivering patient-centered care.
- Ability to work independently while also collaborating effectively within a team environment.
- Demonstrated experience managing chronic conditions in adult or geriatric patient populations is a plus.
