Nurse Practitioner – Primary Care (NP) Summary
- Provides direct patient care to individuals and families in a family medicine clinic setting
- Serves as a clinical expert, delivering comprehensive care and patient education
- Collaborates with a multidisciplinary team to develop and implement care plans
- Focuses on cost-effective care management and optimal resource utilization
- Contributes to continuous quality improvement efforts and organizational excellence
Duties & Responsibilities
- Conduct comprehensive patient assessments, including physical examinations and health histories
- Develop, implement, and evaluate care plans tailored to individual patient needs
- Order and interpret diagnostic tests, prescribe medications, and manage acute and chronic illnesses
- Provide patient and family education on preventative care, treatment options, and health maintenance
- Collaborate with physicians, nurses, and allied health professionals to coordinate patient care
- Monitor patient progress, adjusting care plans as necessary to achieve optimal health outcomes
- Document all patient interactions accurately in electronic medical records
- Uphold clinic policies, procedures, and regulatory requirements to ensure compliance
- Participate in ongoing quality improvement initiatives and professional development activities
- Demonstrate a commitment to patient-centered care and organizational mission

Qualifications & Requirements
- Active and unrestricted licensure as a Registered Nurse (RN) and Nurse Practitioner (NP)
- Nurse Practitioner Furnishing (NPF) license from the California Board of Registered Nursing (BRN)
- Current board certification as NP by a nationally accredited organization in a population focus
- DEA certification to prescribe controlled substances (II-V)
- National Provider Identifier (NPI)
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA)
- Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) from AHA, if assigned to applicable departments
- Primary care experience required
