As a Nurse Practitioner – Primary Care (NP), you will play a pivotal role within our modern, state-of-the-art medical centers. These facilities are equipped with advanced technologies such as cloud-based electronic health records and integrated healthcare systems for seamless care coordination. You will work collaboratively with a diverse care team, including family and internal medicine physicians, RN clinical managers, and center staff, all dedicated to providing outstanding patient care. Nurse Practitioner – Primary Care (NP) Summary
- Full-time position focused on outpatient primary care for seniors and underserved populations
- Work collaboratively in state-of-the-art medical centers equipped with advanced technology
- Supportive, team-based environment alongside physicians, RN clinical managers, and staff

Duties & Responsibilities
- Provide comprehensive outpatient primary care services to a maximum panel of 650 patients
- Average 10–12 patient visits per day, ensuring individualized attention and care
- Collaborate with care team members to deliver coordinated, high-quality care
- Perform in-office medical procedures including minor wound suturing, joint injections, skin biopsies, wound care, and preventive screenings relevant to geriatric patients
- Utilize advanced electronic health record systems for documentation and care coordination
- Engage in quality improvement initiatives and participate in ongoing professional development
- Support the organization’s mission to deliver value-based care and improve patient outcomes

Qualifications & Requirements
- Graduate of an accredited Nurse Practitioner (NP) program
- At least 2+ years of primary care outpatient experience required
- Active, non-probationary state medical license
- Proficiency in in-office procedures such as suturing, joint injections, skin biopsies, wound care, and preventive screenings
