Nurse Practitioner/Physician Assistant – Primary Care (NP/PA) Position Overview
We are seeking dedicated and compassionate healthcare professionals to join our team as a full-time or part-time Nurse Practitioner/Physician Assistant – Primary Care (NP/PA) at a Federally Qualified Health Center (FQHC) in Northern California. This opportunity is ideal for providers who are committed to delivering high-quality, patient-centered care in an outpatient setting while serving an underserved community. As a Nurse Practitioner/Physician Assistant – Primary Care (NP/PA), you will collaborate with a multidisciplinary team to ensure comprehensive care for a diverse patient population.
Key Responsibilities
- Deliver 100% outpatient primary care services to patients of all ages, focusing on preventive care, chronic disease management, and acute illness treatment.
- Conduct patient assessments, develop individualized care plans, and provide health education and counseling to support positive patient outcomes.
- Work collaboratively with 4 physicians and 5 Advanced Practice Providers to manage patient panels and ensure coordinated care delivery.
- Maintain a patient volume of 18–20 visits per day, ensuring a balance between quality care and productivity.
- Adhere to evidence-based clinical guidelines, documentation standards, and regulatory requirements specific to the FQHC environment.
- Participate in team meetings, quality improvement initiatives, and ongoing professional development activities.
- No call responsibilities are required for this Nurse Practitioner/Physician Assistant – Primary Care (NP/PA) role.
Preferred Qualifications
- Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.
- Certification by a nationally recognized body (AANP, ANCC, or NCCPA).
- Experience in primary care, family medicine, or community health settings is preferred, though not required.
- Strong commitment to serving diverse and underserved populations with a focus on health equity.
- Excellent communication, organizational, and clinical skills.
Work Schedule
- Full-time schedule is typically 40 hours per week, often structured over four days.
- Part-time options are available, with a minimum commitment of 30 hours per week.
