Palm Health Resources is seeking a dedicated Nurse Practitioner – Palliative Care NP to join our highly respected interdisciplinary adult palliative care program in Southeast Alabama. As an integral member of our team, you will play a pivotal role in enhancing the quality of life for patients with serious illnesses by providing advanced palliative care services within an inpatient setting. The Nurse Practitioner – Palliative Care NP will work closely with a team that includes hospitalist physicians, fellow nurse practitioners, seasoned case managers, and a spiritual care director. Your role involves comprehensive patient assessments, aggressive symptom management, and coordination of care for hospitalized patients facing life-limiting illnesses. In this collaborative atmosphere, you will have the support and resources necessary to make a significant impact in the lives of both patients and their families.
In this position, you will provide direct and indirect patient care, focusing on symptom relief, medication management, and holistic support. Consultations will occur across various inpatient units, including critical care, oncology, neurology, cardiology, and general medical and surgical departments. The Nurse Practitioner – Palliative Care NP will collaborate closely with referring providers to coordinate individualized care plans, ensuring that each patient’s treatment is aligned with their personal values and goals.
Key responsibilities include initiating and facilitating goals-of-care discussions with patients and their families, guiding them through complex medical decisions, and advocating for patient wishes throughout the continuum of care. This role is strictly inpatient, with no outpatient responsibilities. The work schedule is Monday through Friday, 8:00 am to 5:00 pm, with no call or weekend duties, ensuring a balanced work-life dynamic.

Nurse Practitioner – Palliative Care NP Summary- Full-time, inpatient palliative care nurse practitioner position
- Work within an interdisciplinary team to provide advanced palliative care to adult inpatients

- Lead goals-of-care discussions with patients and families to align treatment with patient preferences
- Document care plans and patient progress accurately and thoroughly
- Participate in interdisciplinary team meetings and case conferences
- Engage in continuing education and professional development activities relevant to palliative care

- Current, valid Nurse Practitioner (NP) license
- Experience in palliative care or willingness to obtain relevant experience
- Desire or eligibility to obtain Advanced Certified Hospice and Palliative Nurse (ACHPN) certification preferred
