Nurse Practitioner (NP) - Summary
- Full-time Nurse Practitioner (NP) position focused on value-based care for Medicare patients

Duties & Responsibilities
- Provide comprehensive primary care to a panel of Medicare patients
- Conduct regular and diagnostic patient visits (average 13-16 patients per day)
- Participate in preventive care initiatives and chronic disease management
- Collaborate with a multidisciplinary care team including scribes, medical assistants, and social workers
- Utilize on-site laboratory services and electronic medical record systems (Canopy and Greenway)
- Engage with community resources including transportation and wellness activities for patients
- Maintain accurate documentation and adhere to value-based care protocols
- Work within a structured weekday schedule (Monday through Friday, 8am-5pm)

Qualifications & Requirements
- Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner certificate program
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Bilingual Spanish-speaking candidates encouraged to apply
- Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
