Duties & Responsibilities
- Conduct thorough patient assessments, physical examinations, and diagnostic evaluations
- Develop and implement individualized treatment plans tailored to patient goals and clinical needs
- Manage chronic conditions, including but not limited to diabetes, COPD, CHF, and dementia
- Provide ongoing patient and family education on medications, nutrition, prevention, and disease progression
- Prescribe medications as authorized within the nurse practitioner’s scope of practice
- Document all patient encounters accurately to support risk adjustment and compliance
- Coordinate care with other providers, leveraging specialty support and resources
- Deliver care in the clinic, patient homes, and via telehealth as needed
- Participate in quality improvement and value-based care initiatives

Qualifications & Requirements
- Master’s degree in Nursing from an accredited institution
- Current board certification as a Nurse Practitioner (NP) in Adult, Adult-Gero, or Family specialties
- Active and unrestricted NP license (state-specific requirements apply)
- DEA license or eligibility to obtain
- At least 2 years of outpatient primary care experience as a nurse practitioner
- Demonstrated ability to practice autonomously in a clinical setting
- Experience with value-based care models preferred
- Proficiency in electronic medical record (EMR) systems; Athena experience highly preferred
- Prior experience with geriatric care, chronic disease management, wound care, injections, ABIs, or imaging interpretation is preferred
- New graduates are not eligible for this role
