Nurse Practitioner (NP) – Primary Care Summary
- Deliver value-based outpatient primary care to a geriatric population
- Manage a small patient panel of 12-16 patients per day
- Work collaboratively with an interdisciplinary team including medical assistants, schedulers, referral specialists, social workers, and pharmacists
- Access to additional specialty support such as endocrinology and podiatry
- Provide care in clinic, patient homes, and via telehealth
Duties & Responsibilities
- Conduct comprehensive patient assessments, physical examinations, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans that align with each patient’s goals
- Educate patients and their families on medication management, nutrition, disease prevention, and health maintenance
- Collaborate closely with an interdisciplinary care team to ensure seamless, coordinated care
- Prescribe medications within the applicable state scope of practice
- Maintain accurate, thorough documentation aligned with risk-adjustment standards
- Provide care in various settings, including the clinic, patient homes for occasional visits, and through telehealth platforms as needed
- Participate in limited call rotation, with the majority of first-call responsibilities handled by a dedicated call center

Qualifications & Requirements
- Master’s degree in Nursing from an accredited institution
- Current board certification as a Nurse Practitioner (Adult, Adult-Gero, or Family)
- Minimum of 2 years of primary care outpatient experience required
- Active Nurse Practitioner license in the state
- DEA registration or eligibility
- Experience with value-based care models preferred
- Demonstrated ability to practice autonomously
- Experience in geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Proficiency with electronic medical records (EMR), with Athena experience highly preferred
- Recent graduates are not eligible for this position
