Duties & Responsibilities
- Conduct patient assessments, physical exams, and diagnostics for older adults.
- Manage chronic conditions including diabetes, COPD, CHF, dementia, and debility.
- Develop and implement individualized treatment plans in alignment with patient goals.
- Educate patients and families on medications, disease progression, nutrition, and preventive care strategies.
- Collaborate with a multi-disciplinary team to ensure integrated care coordination.
- Prescribe medications within scope and in accordance with state licensure requirements.
- Provide primary care services in clinic, at patient homes, and via telehealth as needed.
- Maintain accurate and thorough documentation in compliance with risk-adjusted care standards.

Qualifications & Requirements
- Master's degree in Nursing from an accredited institution.
- Board certification as a Nurse Practitioner (Adult, Adult-Gero, or Family NP).
- Minimum of two years' outpatient primary care experience required.
- Experience in value-based care models preferred.
- Ability to practice independently and autonomously.
- Experience with geriatric patients, chronic disease management, wound care, injections, ABIs, or imaging interpretation is preferred.
- Active Nurse Practitioner license and DEA license (or eligibility).
- Proficiency with Electronic Medical Records (EMR); experience with Athena EMR highly desirable.
- New graduates are not eligible for this opportunity.
