Duties & Responsibilities
- Provide primary care services to a panel of senior patients, focusing on managing chronic conditions and preventative care.
- Conduct patient assessments, diagnose illnesses, develop treatment plans, and prescribe medications as appropriate within the NP scope of practice.
- Manage a patient panel averaging 12-14 patients per day, ensuring personalized attention and quality care for each individual.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral support staff, to coordinate comprehensive care and follow-up.
- Utilize a value-based care approach that prioritizes patient outcomes and wellness, including structured wellness offerings and transportation assistance for patients facing barriers to care.
- Participate in an extremely light, shared call rotation to support patient needs outside of regular clinic hours.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational policies.
- Engage in ongoing professional development and participate in team meetings or quality improvement initiatives as required.
- Communicate effectively with patients, families, and multidisciplinary team members to ensure coordinated, holistic care.
Salary & Benefits
- Competitive base salary starting at $130,000 or higher, depending on experience and qualifications.
- Bonus opportunities of up to 10% of base salary, based on value-based care incentives and patient outcomes.
- Total potential compensation exceeding $143,000, reflecting both base pay and incentive bonuses.

Qualifications & Requirements
- Current and unrestricted Nurse Practitioner (NP) license.
- Board certification as a Nurse Practitioner in a relevant specialty (such as family, adult-gerontology, or primary care).
- Demonstrated experience in delivering primary care, preferably with a focus on senior or chronic care management.
- Ability to communicate effectively in Russian is required to support the patient population.
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Proficiency in electronic medical records and healthcare technology.
