Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Provide comprehensive primary care services to seniors with 3-5 complex diseases such as heart disease, diabetes, and hypertension
- Manage a patient panel with a maximum size of 650, ensuring continuity and quality of care
- See an average of 10-12 patients per day, allowing for personalized, attentive care
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and clinical leadership
- Participate in a value-based care model focused on improving patient outcomes, patient satisfaction, and healthcare efficiency
- Perform office procedures including suturing, joint injections, skin biopsies, and basic wound care as necessary
- Maintain accurate and timely medical records in compliance with regulatory standards
- Participate in a call schedule as needed, determined by the number of providers in the market
- Promote patient engagement by supporting wellness activities and community health initiatives

Qualifications & Requirements
- Current, unrestricted, and active Nurse Practitioner (NP) license
- Minimum of 2 years of outpatient primary care experience required
- Experience in geriatrics is desirable but not required
- Current DEA registration
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Commitment to patient-centered care, quality improvement, and compliance with HEDIS measures
- Strong communication skills and ability to work collaboratively within a multidisciplinary team
