As a Nurse Practitioner (NP) in our primary care team, you will provide comprehensive care to older adults, employing a value-based approach that prioritizes patient outcomes and satisfaction. You will work with a dedicated care team including Medical Assistants, Schedulers, Referral Specialists, Social Workers, and Pharmacists, supplemented by additional specialty support such as Endocrinology and Podiatry. The Nurse Practitioner (NP) will manage a small patient panel, ensuring each patient receives thorough and personalized care. You will be responsible for chronic disease management, patient education, and the development of individualized treatment plans.
This role offers a consistent weekday schedule with no nights or weekends required, promoting an excellent work-life balance. Occasional home visits and telehealth support may be necessary, allowing flexibility in care delivery and accommodating patient needs. The Nurse Practitioner (NP) will also participate in a limited call rotation, with first-line triage managed by a call center to minimize after-hours disruptions. Nurse Practitioner (NP) – Primary Care Summary
- Full-time Nurse Practitioner (NP) role focused on value-based primary care for geriatric patients
- Outpatient clinic setting with occasional home visits and telehealth support

Duties & Responsibilities
- Conduct thorough patient assessments, physical exams, and diagnostic evaluations
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans aligned with patient goals
- Educate patients and families on medications, nutrition, disease progression, and preventive care
- Collaborate with care team members including MA, scheduler, referral specialist, social worker, and pharmacist
- Prescribe medications within regulatory guidelines and state scope of practice
- Deliver care in the clinic, at patient homes, and via telehealth as needed
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in quality improvement and risk-adjustment initiatives

Qualifications & Requirements
- Master’s degree in Nursing
- Board certification as a Nurse Practitioner (NP) in Adult, Adult-Gero, or Family specialties
- Minimum of 2 years’ experience in primary care outpatient settings
- Experience with value-based care preferred
- Ability to practice autonomously
- Active state-specific NP license
- DEA certification (or eligibility)
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation preferred
- Proficiency with EMR systems; Athena experience highly preferred
- No new graduates considered for this role
