We are seeking an experienced and dedicated Nurse Practitioner (NP) to join our value-based primary care team. This full-time position is focused on providing comprehensive, patient-centered care to older adults in a supportive, collaborative outpatient setting. The Nurse Practitioner (NP) – Primary Care will play a vital role in delivering high-quality healthcare, managing chronic conditions, and fostering strong patient relationships. As a Nurse Practitioner (NP) – Primary Care, you will work with a small patient panel, supported by an integrated care team that includes a medical assistant, scheduler, referral specialist, social worker, and pharmacist. Additional specialty support is available in endocrinology and podiatry. The role emphasizes a Monday–Friday daytime schedule with no nights or weekends, supporting an excellent work-life balance. Limited call responsibilities are included, with the call center acting as the first point of contact, ensuring that Nurse Practitioners are rarely contacted after initial triage.

The Nurse Practitioner (NP) – Primary Care is responsible for conducting thorough patient assessments, managing both routine and complex chronic conditions, and developing individualized care plans that align with each patient’s goals. The position requires proficiency in educating patients and families on medications, disease progression, and preventive care, as well as the ability to collaborate with an interdisciplinary team for optimal care coordination.

Nurse Practitioner (NP) – Primary Care Summary- Full-time position focused on value-based outpatient primary care
- Small patient panel (12–16 patients per day)

- Master’s degree in Nursing
- Board-certified Nurse Practitioner (Adult, Adult-Gero, or Family)
- Minimum 2 years’ experience in outpatient primary care (required)
- Experience with value-based care (preferred)
- Ability to practice autonomously
- Active state-specific Nurse Practitioner license
- DEA certification (or eligibility)
- Experience in geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation (preferred)
- Proficiency with electronic medical records (EMR); Athena experience highly preferred
- New graduates are not eligible for this position
- Experienced, autonomous, and motivated Nurse Practitioner (NP)
